Empowering Local Health Program

 

What is the Empowering Local Health Program?

The program is an initiative funded by the Australian Commonwealth Government and Darling Downs and West Moreton Primary Health Network (DDWMPHN). It is designed to improve accessibility of care for priority groups within the Somerset and Southern Downs Regions for:

  • Increasing self-efficacy in the management of chronic conditions

  • Decreasing demand for treatment services to manage chronic conditions and functional decline

  • Decreasing potentially preventable hospitalisations relating to chronic conditions

    Our Priority Groups Include:

  • Aboriginal and Torres Strait Islander peoples

  • People from culturally and linguistically diverse background

  • Older Australians

  • Carers of people with chronic conditions

  • People experiencing socio-economic disadvantage

  • People living in remote, or rural and regional locations

  • People living with disability

  • People with mental illness

  • People who are or have been incarcerated

Our delivery of services are designed to be responsive to you or your clients particular needs in order to effectively address chronic conditions by providing:

• Culturally safe and appropriate services

• Accessible health services that are effective, high-quality, affordable; and

• Flexible service options.
